HOW WE MAY USE AND DISCLOSE PROTECTED HEALTH INFORMATION ABOUT YOU
We may use and disclose protected health information ("PHI") to you, to your personal representative, and for treatment, payment, and certain business activities called "health care operations." Some examples of how we may also use and disclose PHI without your authorization include when we disclose for public health reasons; to report abuse, neglect, or domestic violence; for health oversight activities; for lawsuits and other legal proceedings; for research; to avert a serious threat to health or safety; for specialized government functions such as military or national security purposes; and for workers’ compensation.

All other uses and disclosures of protected health information require your authorization. We will also follow North Carolina law and other federal law that give you more protection of your PHI. For example, North Carolina law gives you more protection of some kinds of PHI including information about communicable diseases, mental health, developmental disability, substance abuse, and pharmacy prescriptions.
