Accident Claim Forms

High Point Regional Health System needs complete information regarding the insurance company name, address, and agent in order to file an insurance claim. Please be sure the responsible party has been notified if the accident occurred on someone else’s property.

Payment is expected within 45 days after your insurance has been filed. If insurance benefits are not received within this time, you will be notified and asked to make suitable payment arrangements according to our Credit and Collection Policy. We do not carry our patient’s debts for extended periods of time and expect the account to be paid in full in 120 days from the date of discharge.
